We invite you to share your story of inspiration with those who are newly diagnosed or those who are looking for answers from a fellow survivor. Submitting your story is very easy – just complete and return the following items to BCAN:

1) A completed questionnaire (this can be mailed, e-mailed, or faxed to us)
2) A completed release form (this must be either mailed or faxed to us)
3) A digital photo of yourself (e-mail is preferable, but you can send via mail and we can scan)
